Your Initial Visits Upon your arrival, please check-in with our reception area so that our receptionist may greet and assist you. If you have not previously completed the new patient forms for you or your child, you will be presented this information where we will appreciate your cooperation in thoroughly completing the requested information so that we may better serve you. If you have orthodontic insurance coverage, please bring your card so that we may assist you in filing for reimbursement from your insurance carrier. There is no charge for this initial visit. After completing your check-in process, you will be taken on a tour of our office and escorted by one of our friendly staff members to a comfortable room where the Doctor will meet you to review your patient dental and medical history forms and perform an oral examination. We will then be able to discuss your possible treatment options for you or your child. At this first visit, our objective is to identify the long-term orthodontic needs for you or your child. The Doctor will determine one of these possible outcomes for treatment: . Treatment is recommended now . Treatment is not recommended at all . Treatment will be needed in the future, but the timing of such will depend upon close monitoring of growth and development, tooth eruption, or other factors We will review all findings in detail with you and discuss any questions or areas of concern you may have such as cost or expected length of treatment. Financing options are also discussed, and any questions you might have can be answered. Please note there are no fees for this initial "New Patient" appointment. We want to demonstrate our level of commitment to you, your smile, and your health by taking the time to give you all the important information needed prior to making the best decision for you and your family. Initial Records Visit If treatment is recommended, we will want to take initial diagnostic records for you to be used for a thorough evaluation of your smile and bite. The Initial Records consist of a number of digital photographs, orthodontic x-rays, and models of the teeth to compliment your initial orthodontic examination. Your prior input along with the information obtained at the initial examination and these diagnostic records will be used to formulate a unique treatment plan for you or your child. Consultation At the consultation visit, the Doctor will sit down with you and take time to review your or your child's case specifically, outlining the personalized planned treatment options which she will thoroughly explain in detail. Also at the consultation visit, our convenient financing options are discussed and we will make sure you have all your questions answered. Print Patient Information Forms Please complete the patient information form, which can be downloaded and printed using the link below (requires Adobe Acrobat). Our staff can also mail these forms to your home when you call to schedule your first visit. Completing this form prior to your arrival facilitates beginning your first visit without delay.
